It's not as disturbing as A Serbian Film, but I recently watched Tideland, that was rather disturbing.
 
The Seasoning House. A bit of a thriller about sex trafficking. Its not the greatest movie in the world, but it did make an impression on me. Because I watched that, Netflix recommended Eden (Abduction of Eden), which is based on a true story of a girl who was kidnapped into sex trafficking for two years. Powerful story, though not particularly disturbing.

Years ago I went to see the first Tremors movie with my boyfriend of the time. He totally freaked out every time the worms were on screen. Said it was one of the creepiest things he'd ever seen. I'm thinking there was something buried in his past on that one since my 6yo saw it last year (not my doing, her dad let her watch it!) and she laughed all through the whole thing.
 
I'm not sure of the name, but I saw it at a drive-in years ago. They were growing babies in the back yard like Cabbage Patch dolls. It definitely was f*d up/disturbing!
 
Mum and Dad 2008 sick freaky and twisted loved it especially the scene where the Dad is having sex with someone's organs in front of Mum and the kids......fucking psychotic!!!
 
Today I watched Citizen X, the Andrej Chikatilo story with Donald Sutherland and Max Von Sydow. Not particularly disturbing since there wasnt any real blood and guts, but you can hardly watch a movie about that sick fuck and not be disturbed.
 
The Englishman Who Went up a Hill and Came Down a Mountain. The whole movie is about a bunch of semi-retarded Welsh villagers piling dirt onto a hill so they can claim to have a mountain. THE WHOLE FUCKING MOVIE!
Storyline
Two English cartographers visit the small South Wales village of Ffynnon Garw, to measure what is claimed to be the "first mountain inside of Wales". It's 1917, and the war in Europe continues. The villagers are very proud of their "mountain", and are understandably disappointed and furious to find that it is in fact a "hill". Not to be outwitted by a rule (and the Englishmen who enforce it), the villagers set out to make their hill into a mountain, but to do so they must keep the English from leaving, before the job is done.

mr cb and I watched Cannibal Holocaust and I yelled "TOUCHDOWN!" every time someone swung a machete or took a bite of long pig...But the original Last House on the Left freaked me out so badly I didn't go the movies for a solid year.

Martyrs fucked with me for a few days, LHOTL was pretty fucked up too. American History X was pretty disturbing to me too, idk if I'm the only one on that one....
 
Don't get me wrong, it is a really good movie, you'll probably ask yourself "What the fuck did I just watch??" Don't be afraid though :)
 
Crap, I just ruined the whole thing for myself by looking it up on Wikipedia. Martyrs is one fucked up movie!
 
When Strangeland first came out, I was about 16. I watched that and it scared the crap out of me. I had nightmares for a long time after that. Thankfully I don't remember much about it now and will never watch it again!
 
Tideland was mentioned above. I found it pretty disturbing as well, and incredibly sad. The book is even better, although the movie was directed by Terry Gilliam who is one of my absolute favorite directors. I highly recommend both the book and the film.

And Anndroyd10, I loved Strangeland. I love Dee Snider, which may be part of the reason why I enjoyed that movie so much.
 
I haven't gone through the whole thread... I have watched Salo, a Serbian film, Cannibal holocaust you name it I have seen it, but even though its tame in comparison Martyrs disturbed me. Threads (pretty tame as well) also got to me except for the fact they went 10 years into the future... I mean cmon 10 years and they couldn't rebuild and no other country would help lol. The girl next door bothered me too. Human centipede two bothered me for weeks.

I have this movie on dvd. I also have Silence of the Lambs and Man Hunter( the Red Dragon). I always had a fascination with the serial killer movies and true crimes. Suspect Zero was another one I really liked.
 
The one that stands out to me as the most disturbing was the movie Bundy. Specifically in his "rape shack" and he has one woman tied up and while he's raping the other he smashes her head in with a big rock.
And then he has sex with the bodies in the woods as they decompose. This movie changed the way I viewed human nature.
